Work Description:
This position is responsible for assuring that patients have access to quality, cost-effective healthcare and will assist in the assessment, coordination and monitoring activities necessary to accomplish this. They are also responsible for assisting with the identification, screening and assessment of potential case management cases, developing specific goals, interventions and recommendations, providing reports to clients, and representing company in the healthcare community. This is a telecommute position

Qualifications and Requirements for this Miscellaneous Category position:
Requirements:
Bachelors degree in nursing, healthcare or related field
RN with current, active and unrestricted license Washington
CCM with current active and unrestricted certification (required for CM II position)
Four years of clinical experience
2-5 years experience in home health, discharge planning, case management, utilization review, insurance industry and/or recent nursing experience
Computer literacy with working knowledge of MS Office and case management software
Self-motivated, able to work under pressure, effectively manage multiple priorities, meet deadlines and function well in a team environment
Excellent communication skills
Education or experience may substitute for education and/or experience
